Curative members can receive covered telehealth therapy services with no out-of-pocket cost — $0 copays, $0 coinsurance, and $0 deductibles on most plans. To verify your specific coverage, please contact your employer's benefits administrator or check your Curative member ID card.
OUT OF NETWORK
If you'd like to file for out of network reimbursement from your insurance company, you may pay out of pocket and I will provide you with a Super Bill receipt. You also have the right to receive a Good Faith Estimate if you do not have insurance or are not using it. I am not in network with Medicare or Medicaid.
